This non-fillable version of the model Chapter 13 plan form may be used only by pro se debtors and others not required to file electronically. If a debtor has more than two creditors to list in the same paragraph on the form, use an overflow sheet to list the additional creditors. All information required for the treatment of the creditor on the plan form should be included in the box on the overflow sheet. Make sure to note the applicable paragraph number for the treatment of the creditor on the overflow sheet.
