Effective Monday, September 29, 2014, the unclaimed funds/small dividends case filing event in the court’s Case Management/Electronic Case Filing System (CM/ECF) will be modified into two separate filing events. The specific name of the petition (unclaimed funds or small dividends, but not both) should be clearly identified in the title of the petition. In addition to the change of events in ECF, the Application for Payment of Unclaimed Funds or Small Dividends located on our internet site is also being amended. Claimants will be required to submit a W9 (Request for Taxpayer Information) form in addition to other requested documents when submitting an application.
